Job Description:

Conducts comprehensive psycho-social assessments to evaluate patients' mental health and substance use disorder needs.
Develops individualized care plans based on assessment findings, incorporating patient goals and preferences.
Provides counseling and therapeutic support to patients to address mental health and substance use issues.
Connects patients with community resources and support networks to aid in their recovery and reintegration.
Collaborates with a multi-disciplinary team to coordinate care and ensure holistic treatment approaches.
Educates patients and families about substance use disorders, mental health conditions, and available treatment options.
Monitors patient progress and adjusts care plans as needed to support ongoing recovery.
Facilitates group therapy sessions to provide peer support and shared experiences.
Assists patients in navigating healthcare systems and accessing necessary services.
Advocates for patients' needs and rights within the healthcare system and community.
Maintains accurate and confidential patient records to document assessments, care plans, and progress.
Identifies and intervenes for suspected abuse, including sensitivity to age-specific concerns.
Demonstrates working knowledge of the involuntary commitment process.
Provides crisis intervention and support during acute episodes of mental health or substance use issues.
Performs other duties as assigned.

Excellent interpersonal and written communication skills [Required]
Ability to act in an autonomous, self-directed manner while maintaining the ability to collaborate with other members of the team [Required]
Excellent communication, negotiation and advocacy skills with patients, families, team members, health care team and community resources [Required]
Working knowledge of community and treatment resources based on age and clinical need [Required]
