Job Description:

  • Conducts comprehensive psycho-social assessments to evaluate patients' mental health and substance use disorder needs.
  • Develops individualized care plans based on assessment findings, incorporating patient goals and preferences.
  • Provides counseling and therapeutic support to patients to address mental health and substance use issues.
  • Connects patients with community resources and support networks to aid in their recovery and reintegration.
  • Collaborates with a multi-disciplinary team to coordinate care and ensure holistic treatment approaches.
  • Educates patients and families about substance use disorders, mental health conditions, and available treatment options.
  • Monitors patient progress and adjusts care plans as needed to support ongoing recovery.
  • Facilitates group therapy sessions to provide peer support and shared experiences.
  • Assists patients in navigating healthcare systems and accessing necessary services.
  • Advocates for patients' needs and rights within the healthcare system and community.
  • Maintains accurate and confidential patient records to document assessments, care plans, and progress.
  • Identifies and intervenes for suspected abuse, including sensitivity to age-specific concerns.
  • Demonstrates working knowledge of the involuntary commitment process.
  • Provides crisis intervention and support during acute episodes of mental health or substance use issues.
  • Performs other duties as assigned.
Knowledge, Skills, and Abilities:
  • Excellent interpersonal and written communication skills [Required]
  • Ability to act in an autonomous, self-directed manner while maintaining the ability to collaborate with other members of the team [Required]
  • Excellent communication, negotiation and advocacy skills with patients, families, team members, health care team and community resources [Required]
  • Working knowledge of community and treatment resources based on age and clinical need [Required]
  • Critical-thinking/problem-solving skills [Required]
  • Ability to utilize in-house and external resources [Required]
  • Flexibility in prioritization and effective organizational skills [Required]
  • Ability to utilize stress management techniques effectively [Required]
  • Skills in advocacy [Required]
  • Ability to provide solution-focused theory, cognitive behavioral theory and systems theory [Required]
  • Effective computer skills, particularly Microsoft Office Outlook, Word, Excel [Required]

Education:
  • Bachelor's [Required]

Field of Study:
  • in Behavioral Health

Work Experience:
  • 2+ years’ experience in social work or behavioral health [Required]
  • Experience in acute care hospital [Preferred]
  • Experience working with medically complex patients [Preferred]
  • Experience working with substance use and mental health disorders [Preferred]
