Position: LMSW (Licensed Master Social Worker) / MSW

Schedule

• 4x10-Hour Day Shifts (40 hours/week)
• Shift Timing: 07:30 AM – 06:00 PM
• Weekend Requirement: Every other weekend
• Guaranteed Hours: 70 hours per 2 weeks

Position Summary

We are seeking an experienced LMSW / MSW with a strong background in acute care case management. The ideal candidate will bring expertise in discharge planning, care coordination, and the ability to work across multiple hospital units in a fast-paced healthcare environment.

Key Responsibilities

• Perform discharge planning and care coordination
• Collaborate with multidisciplinary healthcare teams
• Assess patient needs and develop individualized care plans
• Facilitate transitions of care across various settings
• Work across multiple units, including Med-Surg and Emergency Department (ED)
• Document patient information using EPIC EMR
• Ensure compliance with healthcare policies and procedures

Required Qualifications

• Active Washington State LMSW license (pending accepted)
• Minimum 1 year of relevant experience
• Acute care experience required
• Experience in discharge planning and care coordination
• BLS certification required

Preferred Qualifications

• Experience in Med-Surg, Emergency Department, or Length of Stay (LOS) management
• Strong adaptability with the ability to float between units

• Floating required across units and campuses
• Charting System: EPIC
• Parking provided/paid by facility
• COVID-19 vaccination not required
