Social Worker Assistant for Rehab/Long Term Care Facility.

Duties, Requirements and Responsibilities:

Directs, establishes, and plans the overall policies and goals for the Social Worker Department

Responsible for patient assessments, care planning and helping the patients and families adjust to their new surroundings

Discharge Planning

Plans and administers social service programs

Reports to Director of Social Services

Provides input to strategic decisions that affect the functional area of responsibility.

Capable of resolving escalated issues arising from operations and requiring coordination with other departments.

Must have at least 2 years' experience in long term care.
