Job Title: Client Care Coordinator (Senior Care Advocate)

Location: Burlington County and Cherry Hill, NJ

Experience Level: 2+ Years Home Health/Home Care Sales or Coordination

THE OPPORTUNITY

We are seeking a high-caliber Client Care Coordinator who understands that Home Care is a specialized partnership, not a generic service. PRIMARY RESPONSIBILITIES

Client Intake & Service Coordination - Lead the client journey from initial inquiry to service initiation by conducting expert intake assessments and in-home evaluations that build immediate trust.

Service Planning & Oversight - Ensure reliable shifts and quality care

Scheduling & Staffing - Mastermind strategic staffing by matching qualified caregivers to client needs and managing real-time schedule updates for seamless care continuity

Documentation & Compliance - Ensure total operational compliance by maintaining accurate, timely, and confidential documentation in accordance with agency standards.

Client Relationship Management - Drive client loyalty through proactive family communication, rapid concern resolution, and strategic follow-up visits to ensure service excellence.

Office & Operational Support - Uphold service continuity and agency standards by managing daily operations, quality assurance monitoring, and mastery of our specialized schedulingsoftware.

Culture & Communication - Foster a culture of excellence by championing agency values and driving professional collaboration across the entire care team.

GENERAL QUALIFICATIONS

  • Clinical Proficiency: Deep understanding of ADLs, IADLs, and the nuances of Dementia/Alzheimer’s care.
  • Sales Sophistication: Proven ability to handle "Price" and "Privacy" objections using value-based rebuttals.
  • CRM Mastery: Experience managing lead flows to ensure no family is left without a follow-up.
  • Communication: Specialized experience in navigating the emotional guilt and stress of adult children.

EDUCATION & EXPERIENCE

  • Associate degree or equivalent required; bachelor’s degree preferred
  • Minimum 2 years of healthcare or home care experience preferred
  • Prior supervisory or coordination experience preferred
  • Equivalent combination of education and experience may be considered

CERTIFICATIONS & REQUIREMENTS

  • Valid driver’s license required
  • Current auto insurance required
  • Additional certifications as required by agency policy or state regulations

  • Ability to assess situations and make sound decisions
  • Strong critical thinking skills in dynamic care environments
  • Ability to work independently and as part of a team

PHYSICAL DEMANDS & WORK ENVIRONMENT

  • Frequent communication (verbal and written) required
  • Frequent use of hands for computer and documentation tasks
  • Occasional standing, walking, sitting, and lifting up to 50 lbs
  • Work performed in office and client home settings
  • Potential exposure to illness or bodily fluids in home care environments
  • Moderate noise level

Pay: $52,000.00 - $58,000.00 per year
