We are seeking a dedicated and compassionate Registered Nurse - Intake Coordinator to join our healthcare team. Responsibilities
- Conduct comprehensive patient assessments during the admissions process, including medical history and physical. .
- Collaborate with interdisciplinary teams to develop individualized care plans tailored to each patient's needs.
- Ensuring adherence to safety protocols and best practices.
- Provide education to patients and families regarding treatment plans, medication administration, and health management strategies.
- Maintain compliance with HIPAA regulations while handling sensitive patient information.
- Participate in discharge planning and follow-up care coordination for patients transitioning out of the facility.
- Engage in ongoing professional development to stay current with best practices in nursing care.
Qualifications
- Current Registered Nurse (RN) licensure in the state of practice.
- Experience in acute care settings, psychiatric hospital, or long-term care preferred.
- Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), and medical documentation practices.
- Strong assessment skills with the ability to recognize changes in patient conditions promptly.
- Previous experience working with diverse populations, including geriatrics.
- Knowledge of pain management techniques, infection control practices, and behavioral health considerations is beneficial.
