Social Worker

We are seeking a compassionate and dedicated Social Worker to join our team at a skilled nursing facility. The ideal candidate will be passionate about advocating for residents, supporting families, and helping ensure the emotional and psychosocial well-being of those in our care.

Responsibilities:

• Assist residents and families with admissions, discharge planning, and transition of care
• Provide emotional support and counseling to residents and families
• Coordinate care plans with the interdisciplinary team
• Help residents access community resources and services
• Maintain accurate documentation in compliance with facility and regulatory requirements
• Address resident concerns and assist with conflict resolution when needed
• Participate in care conferences and support resident rights initiatives

Qualifications:

• Degree in Social Work or related field preferred
• Previous experience in long-term care or healthcare setting preferred
• Strong communication and interpersonal skills
• Compassionate, organized, and team-oriented
• Knowledge of discharge planning and resident advocacy preferred
