If so, 1st Care Palliative and Hospice Services PA is looking for a dedicated Social Worker to join our team in Allentown, PA. As a Social Worker in our hospice and home health programs, you will have the opportunity to provide essential support and guidance to patients and their loved ones during a difficult time.

Daily Responsibilities:

• Conduct psychosocial assessments to identify patient and family needs
• Connect patients and families with community resources for additional support
• Provide supportive counseling and advocacy for clients
• Collaborate with interdisciplinary teams to develop comprehensive care plans
• Offer grief and loss counseling to individuals and families

Required Skills:

• Experience in medical social work or hospice care
• Strong understanding of biopsychosocial assessment techniques
• Ability to provide client advocacy and supportive counseling
• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
• Knowledge of community resources and services

Nice to Have Skills:

• Experience in Home Health and Hospice
• Family counseling and bereavement counseling expertise
• Anger management and mental health counseling skills
