Care Manager

Aging Advocates is seeking an experienced and compassionate Care Managerto join our team. This role is ideal for professionals who have a strong background working with seniors and are passionate about care coordination, advocacy, and family support.

Responsibilities

• Conduct comprehensive evaluations for older adults
• Develop and oversee individualized care plans
• Coordinate medical, social, and community-based services
• Advocate for clients and families across care settings
• Maintain accurate documentation and client case notes
• Collaborate with healthcare providers, families, and community partners

Qualifications

• Required: Experience working with seniors
• Background in care management, case management, or social services
• Strong organizational, communication, and critical-thinking skills
• Ability to manage a caseload independently
• Professional, compassionate, and client-focused approach
