Overview
We are seeking a compassionate and dedicated person for a temporary position on the Social Work team. The ideal candidate is either currently matriculating in a social services or related field or has experience in social work, preferably in a skilled nursing setting. This role requires a commitment to supporting individuals and families in navigating challenges related to spending time in a rehabilitative or long term care setting. The Social Worker will work collaboratively with the interdisciplinary team to achieve optimal outcomes for all patients.

Responsibilities
- Interviews resident and/or designated representative for the purpose of completing a psychosocial history and assessment of the resident, identifying problem areas, and developing care plans, goals and discharge plans. Reviews each resident no less than once every three (3) months, and as appropriate, revises the resident’s social work assessment to assure continued accuracy of the assessment
- Informs residents of their rights and responsibilities including information regarding DNR orders, Health Care Proxy and Advance Directives
- Provides on-going counseling, as needed, to residents regarding issues related to adjustment to nursing home lifestyle
- Provides crisis intervention, counseling as indicated and responds to social work referrals
- Facilitates needed communication with other disciplines on behalf of the residents, including medical, nursing, dietary, rehabilitation and psychiatric services
- Coordinates and monitors needed available services for individual residents to assure optimum level of emotional, physical and psychosocial well-being and independence
- Involves the resident, other disciplines and Administration regarding matters such as bed retention, room change, transfer and discharge
- Interprets residents’ needs and behaviors and extends professional intervention to all levels of staff, suggesting positive approaches
- Provides bereavement counseling for residents, families and other affected individuals
- Completes and updates the screen instrument in compliance with New York State Department of Health regulations. Responsible for chart notations, MDS section completions and appropriate care planning
- Educates and supports residents in understanding and complying with Infection Control policies and procedures to promote the health and safety of residents and staff
- Coordinates and promotes safe visitation with families
- Performs other duties as directed by the Director of Social Services
- Attends all mandatory in-service education programs
- Assists in the orientation of new employees to the department
Requirements
- Bachelors or Masters Degree in Social Work or related field, LMSW preferred - or currently matriculating in a social work or related degree program
- Excellent written and interpersonal skills
- Ability to work as a member of an interdisciplinary team
- Committed to continuing professional development
- Ability to communicate effectively in English, both oral and written forms
- Bilingual English/Spanish a plus

Pay: $32.00 - $37.00 per hour
