Aging Care Experts is an private Aging Life Care Management practice serving Brevard County. We provide compassionate, client-centered guidance and support to older adults and their families, helping them navigate healthcare, caregiving, aging in place, facility placement, and long-term planning with dignity and clarity.

We are seeking an experienced Aging Life Care Professional to join our team during an exciting period of growth and transition, while continuing the high standard of service our community has come to trust.

Position Summary
The Regional Director will market our services to industry professionals to build a referral network, onboard and assess new clients, and supervise our team of care managers for service delivery. The Care Managers (and the Regional Director) will provide comprehensive care management services to older adults and their families, including assessments, care planning, coordination of services, advocacy, and ongoing monitoring. This supervisory role is ideal for a Registered Nurse or Social Worker with strong clinical judgment, excellent communication skills, managerial experience, and a passion for working with aging populations.

Key Responsibilities

  • Market services to industry providers and the general public
  • Build a network of referral partners
  • Onboard new clients
  • Conduct in-home and facility-based assessments
  • Develop and implement individualized care plans
  • Oversee team of care managers
  • Coordinate medical care, home care, and community services
  • Communicate with families, providers, and legal/financial professionals
  • Advocate for clients within healthcare and long-term care systems
  • Monitor client status and adjust care plans as needed
  • Maintain accurate, timely documentation and reports
  • Participate in on-call rotation

Qualifications

  • Social Work or Nursing degree, Masters degree preferred
  • Minimum 2 years experience in geriatrics, case management, home health, hospice, or related field
  • Knowledge of Medicare, Medicaid, long-term care options, and community resources
  • Strong organizational and problem-solving skills
  • Excellent written and verbal communication
  • Willingness to problem solve and a can-do attitude
  • Excellent interpersonal relationship skills, diplomacy, and collaboration with external service providers
  • Comfortable working independently in the field
  • Valid driver’s license and reliable transportation
  • Aging Life Care Association (ALCA) certification (CCM, CMC, C-SWCM, C-ASWCM) a plus (or willingness to obtain)

  • Bachelor's (Preferred)
