Role Description

This is a full-time hybrid role for a Licensed Master Social Worker located in Brooklyn, NY, with flexibility for some remote work. The Licensed Master Social Worker will provide direct support and resources to patients, addressing their psychosocial and emotional needs. Responsibilities include conducting assessments, developing care plans, offering counseling, connecting patients with community resources, and advocating for patient needs. The role involves collaboration with medical staff to ensure holistic care and promote patient well-being.

Qualifications

• Proficiency in conducting patient assessments, care planning, and case management
• Strong counseling and crisis intervention skills
• Knowledge of community resources and experience connecting patients to services
• Proficiency in clinical documentation and record-keeping
• Excellent communication, interpersonal, and advocacy skills
• Ability to work collaboratively with interdisciplinary teams
• Master's degree in Social Work (MSW) and valid LMSW license in New York State
• Experience in community or healthcare settings is highly desirable
• Understanding of culturally sensitive practices and commitment to diversity, equity, and inclusion
• Proficient in using technology to manage remote and on-site workflows
