Health Home Care Manager-Per diem/Part Time/Full Time-Cantonese Bilingual-home visiting

Asian Community Care Management, Inc is seeking a Per Diem Health Home Care Manager to provide coordination services for Medicaid members. The ideal candidate will have a bachelor’s degree or higher in social work/psychology, nursing, rehabilitation, education, sociology, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing, or other human services field. Health home experience is highly preferred, as well as being an Epic/foothold user.

As a Health Home Care Manager, you will provide telephonic and home visit services to Medicaid members, assisting them in navigating the healthcare system and coordinating their care. Paid training will be provided, and a mentor will be assigned to support you through the entire training process. Our support team will work with you every step of the way to ensure your full satisfaction.

Qualifications:

• Bachelor's degree or higher in social work/psychology, nursing, rehabilitation, education, sociology, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing, or other human services field.
• Health home experience highly preferred.
• Epic/foothold user highly preferred. Experiences in EPIC system is a plus!

Per Diem Pay range: (PM-PM means "per member, per month")

• For 1-10 cases: $90 PM-PM (For 1-5 Harp cases: $135 PM-PM and for 1-3 HH+ cases: $270 PM-PM)
• For 11-20 cases: $110 PM-PM (For 6-10 Harp cases: $155 PM-PM and for 4-6 HH+ cases: $330 PM-·PM)
• For 21 or more cases: $120 PM-PM

***Please note that the nature of this role is flexible and can be further discussed during the interview process. It may begin on a per diem basis, with the possibility of transitioning into a part-time or full-time position depending on evolving needs.***

We need the candidates to handle one of the following locations: Manhattan, Brooklyn, Queens or Long Island
