Licensed Social Worker - Hospice

We are seeking a licensed social worker to join a hospice team serving the Charleston, SC area.

In this role, you will interact with diverse individuals, organize and implement hospice policies, and address issues related to patient care. You will conduct assessments, social evaluations, and develop intervention plans, focusing on family dynamics, caregiver abilities, communication, and high-risk situations such as abuse or neglect.

Qualifications include effective communication skills, integrity, understanding hospice philosophy, and the ability to travel to patient homes and facilities. A South Carolina license, valid driver’s license, and knowledge of Microsoft Office are required.
