Licensed Social Worker - Hospice

We are seeking a dedicated Licensed Social Worker to join our hospice team serving the Charleston, SC area.

In this role, you will

• interact effectively with diverse individuals
• conduct assessments and social evaluations, including family dynamics and caregiver abilities
• identify high-risk situations such as suicide, neglect, or abuse
• develop intervention plans based on evaluation findings
• collaborate as part of the interdisciplinary care team
  
  

Qualifications include

• strong communication and interpersonal skills
• integrity and understanding of hospice philosophy
• ability to travel within the community
• proficiency in Microsoft Office
• South Carolina licensure, valid driver’s license, and auto insurance
