The Healthy Start Care Coordinator is responsible for providing comprehensive, culturally responsive, and family-centered services to pregnant women, infants, and families enrolled in the Healthy Start program. The Care Coordinator delivers evidence-informed interventions through home visits, community outreach, screenings, assessments, education, and referrals. This position ensures that families receive timely support, follow-up, and linkage to community resources to improve maternal and child health outcomes.
Key Responsibilities
Client Engagement & Service Delivery
- Conduct initial outreach attempts, screenings, and enrollment for eligible clients in accordance with Healthy Start Standards & Guidelines (HSSG).
- Complete comprehensive assessments, including risk screenings, ASQ/ASQ-SE2 developmental screenings, and ongoing evaluations.
- Provide individualized care coordination, home visits, health education, interconceptional counseling, and goal-setting using evidence-informed tools.
- Develop and monitor care plans using the PIF model (Plan, Intervention, Follow-Up).
- Ensure documentation of all encounters within 3 business days, maintaining accuracy, completeness, and compliance with program standards.
- Provide maternal and infant health education, including safe sleep, breastfeeding support, family planning, and mental health awareness.
Resource Navigation & Referrals
- Connect clients to appropriate community resources such as WIC, Medicaid, housing support, behavioral health services, and pediatric care.
- Follow up on all referrals to ensure timely access and client satisfaction.
- Maintain updated knowledge of local service providers and community resources.
Family Support & Advocacy
- Build trusting relationships with clients to support engagement, retention, and positive health behaviors.
- Advocate for client needs within the healthcare and social support system.
- Support families in goal-setting and problem-solving related to health, parenting, and safety.
Program Compliance & Administration
- Maintain confidentiality in alignment with HIPAA and program requirements.
- Document all encounters, screenings, referrals, and outcomes within the Well Family System (WFS).
- Meet productivity and caseload expectations, including timely first, second, and third attempts.
- Participate in case conferences, staff meetings, QA/QI reviews, and continuing education.
- Complete monthly reports and contribute to program evaluation activities as required.
Outreach & Community Engagement
- Participate in community events, health fairs, and collaborative initiatives to promote Healthy Start services.
- Foster positive relationships with hospitals, OB/GYN practices, pediatric offices, and community partners.
- Provide community education on maternal and child health as needed.
