Our growing home health agency is seeking a highly organized, proactive, and accountable Care Manager to oversee patient cases, staffing, and daily operations. This role is critical to ensuring continuity of care, strong relationships, and operational efficiency across all assigned cases.

Key Responsibilities
- Staff and restaff patient cases, including coverage for HHA vacations and call-outs
- Oversee and manage assigned Home Health Aides (HHAs) to ensure performance and reliability
- Build, update, and maintain permanent schedules for all assigned cases
- Conduct routine courtesy calls to patients and ensure satisfaction with services
- Monitor EVV/telephony systems in real time to ensure compliance and attendance
- Troubleshoot scheduling gaps and last-minute changes efficiently
- Build and maintain relationships with patients and families
- Collaborate with social workers to generate new patient leads
- Develop and maintain relationships with HMO case managers
- Manage and track patient authorizations to ensure continuity of care
- Maintain accurate and timely documentation in company systems
This role requires strong decision-making, urgency, and the ability to manage multiple priorities at once.
Who We’re Looking For
We are seeking candidates who demonstrate:
✔ Experience in home health, home care, or medical case coordination (preferred)
✔ Strong organizational and multitasking skills
✔ Ability to manage high-volume coordination and real-time scheduling
✔ Bilingual (English / Spanish) — REQUIRED
✔ Excellent communication and relationship-building skills
✔ High level of accountability, reliability, and follow-through
✔ Ability to stay calm, solution-oriented, and efficient under pressure

Pay: $22.00 per hour
