We are seeking an enthusiastic and community-focused Benefits Counselor to join our growing team. This role is ideal for someone passionate about helping individuals navigate healthcare access, community resources, and social drivers of health while building meaningful relationships within the community.
Key Responsibilities
- Assist patients in identifying and addressing barriers to healthcare access and social drivers of health.
- Connect patients and families to community resources, healthcare programs, and support services.
- Coordinate community outreach events, health education activities, and resource initiatives.
- Provide health education and guidance to patients and community members.
- Advocate for community needs by attending community meetings, outreach events, and partnership activities.
- Manage and maintain a caseload of patients requiring ongoing support and follow-up.
- Maintain accurate patient records and documentation using electronic systems.
- Build positive relationships with patients from diverse backgrounds and age groups.
- Collaborate with healthcare providers and interdisciplinary care teams to improve patient outcomes.

- High School Diploma or equivalent required.
- Post-secondary degree in Public Health, Social Work, Psychology, Human Services, or related field preferred.
- Community Health Worker (CHW) Certification preferred.
