Compass Health Services is a rapidly growing home care agency providing Skilled Nursing, Pediatric Private Duty Nursing (PDN), and non-skilled home care services for both adult and pediatric patients throughout Texas. We are committed to delivering exceptional care and building strong relationships with referral partners who trust us to provide timely, reliable Start of Care (SOC) services.

We are seeking an experienced and highly connected Home Care Provider Relations Manager to take full ownership of the Houston-Sugar Land territory and drive immediate census growth.

Position Overview

This is a high-impact role for a seasoned healthcare relationships manager who already maintains strong relationships with referral sources throughout the Dallas-Fort Worth market and can begin generating referrals immediately.

Our service lines include:

  • Skilled Nursing
  • Pediatric Private Duty Nursing (PDN)
  • Non-skilled home care for adults
  • Non-skilled home care for pediatric patients

The ideal candidate has established relationships with:

  • Medicaid Managed Care Organization (MCO) Service Coordinators and Case Managers
  • Skilled Nursing Facility (SNF) Social Workers and Discharge Planners
  • Hospital Case Managers and Care Coordinators
  • Pediatric Clinics and Specialty Physician Offices
  • Primary Care and Specialty Physician Practices
  • Community organizations and healthcare professionals

Your primary responsibility will be to generate qualified referrals and ensure timely, high-quality Starts of Care (SOC) that lead to sustained census growth.

Successful candidates will be expected to:

  • Generate 10–20 qualified referrals per month
  • Achieve 8–15 completed Starts of Care (SOC) monthly
  • Build and maintain an active referral pipeline
  • Submit weekly activity and pipeline reports
  • Consistently meet or exceed census growth goals

Candidates without established healthcare referral relationships in the Dallas-Fort Worth area will not be considered.

Key Responsibilities

  • Develop and maintain referral relationships throughout the Dallas-Fort Worth area
  • Generate referrals for:
  • Skilled Nursing
  • Pediatric Private Duty Nursing (PDN)
  • Non-skilled home care for adults
  • Non-skilled home care for pediatric patients
  • Meet regularly with MCO case managers, SNF discharge planners, physicians, and community partners
  • Coordinate with intake, authorization, staffing, and clinical teams to ensure rapid Start of Care
  • Document all referral activity and follow-up efforts in the company CRM
  • Represent the agency professionally at networking events and industry meetings
  • Provide weekly reports on referral activity, admissions, and pipeline growth

Compensation

  • Base Salary: $70,000-$80,000 per year
  • Commission Structure

Qualifications

  • Minimum 3 years of home care, home health, or healthcare marketing experience
  • Established referral relationships in the Houston and surrounding market
  • Proven track record of generating referrals and growing census
  • Strong knowledge of Texas Medicaid, Managed Care Organizations, and discharge planning workflows
  • Ability to generate referrals within the first 30 days
  • Excellent communication and relationship-building skills
  • Self-motivated, organized, and results-driven
  • Reliable transportation and willingness to travel extensively throughout DFW

Preferred Experience

  • Home Care Administration license
  • Skilled Home Health Marketing
  • Pediatric Private Duty Nursing (PDN) Marketing
  • Non-skilled home care marketing for adult and pediatric populations
  • Experience with Texas Medicaid programs, including STAR and STAR Kids
