Job Title: Part-Time Licensed Master Social Worker (LMSW)

Location: New York, NY 10065

Duration: 27 Weeks

Schedule

• Variable shifts:
  • Monday: 9:30 AM – 4:00 PM
  • Tuesday: 9:00 AM – 5:00 PM
  • Wednesday: 10:00 AM – 3:00 PM
• 24 hours per week (3x8 schedule across Monday–Wednesday)

Position Summary

We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to provide patient-centered social work services in a healthcare setting. The ideal candidate will assess patient needs, support care planning, and help connect patients and families with appropriate resources to enhance overall care outcomes.

Key Responsibilities

• Conduct interviews with patients and families to assess social, psychological, and environmental needs
• Review medical records and collaborate with care teams to determine appropriate interventions
• Provide individual and/or group counseling to patients and families facing personal or environmental challenges
• Connect patients and families with community resources, including financial assistance, counseling, rehabilitation, and medical support services
• Perform discharge planning, including coordinating placement in rehabilitation, extended care, or specialized facilities when needed
• Participate in interdisciplinary team meetings and communicate social and emotional factors affecting patient care
• Support care teams in addressing barriers to treatment and recovery
• Contribute to quality improvement initiatives and program development

Required Qualifications

• Minimum 1 year of experience as a Licensed Master Social Worker (LMSW)
• Strong knowledge of psychosocial assessments
• Experience in medical social work within a hospital or outpatient setting
• Valid Licensed Master Social Worker (LMSW) license issued by New York State Education Department (NYSED)

Education

• Master’s Degree in Social Work (MSW)
