Care Coordinator LPN

We need a compassionate, efficient and understanding LPN to assist with our patients.

Care Coordinator Job Duties:

• Orients and educates patients and their families by meeting them; explaining the role of the patient care coordinator; initiating the care plan; providing educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements.
• Carries out interdisciplinary care plan as outlined by rendering provider. Utilizes case management tools by participating in meetings; coordinating information and care requirements with other care providers; resolving issues that could affect smooth care progression.
• Monitors care plans for progress toward desired care outcomes and works with direct care to expedite scheduling and completion of tests, procedures, and consults
• Participates with interdisciplinary team to evaluate outcomes and intervention effectiveness. Implements team recommendations.
• Respects patients by recognizing their rights; maintaining confidentiality.
• Maintains patient care database by entering new information as it becomes available; verifying findings and reports.
• Contributes to team effort by accomplishing related results as needed.
• May be requested to perform nursing duties as needed.

Job Type: Full-time

Pay: From $23.22 per hour
