Circle of Life Hospice is seeking a compassionate and experienced Part-Time Medical Social Worker with a strong background in home health and/or geriatric care to support patients and families during life’s most meaningful transitions. Position Overview:

The Medical Social Worker is an essential member of the interdisciplinary hospice team, providing psychosocial support, counseling, and resource coordination for patients and their loved ones. This part-time role offers flexibility while making a meaningful impact in end-of-life care.

Your Role:

As a Part-Time Medical Social Worker, you will:

  • Conduct psychosocial assessments and contribute to individualized care planning.
  • Provide emotional support, counseling, and crisis intervention.
  • Assist patients and families with advance care planning and decision-making.
  • Connect families with community resources, including financial, placement, and supportive services.
  • Collaborate with nurses, physicians, chaplains, and the hospice care team.
  • Support bereavement and grief needs as appropriate.
  • Participate in hospice admissions and ongoing patient visits.
  • Uphold our mission that “no one should die alone.”

Qualifications:

  • Master’s Degree in Medical Social Work (MSW) from an accredited program (required).
  • Current Social Work license in the State of Nevada (MSW required per regulations).
  • Minimum of 5 years of experience, preferably in home health, hospice, or geriatric care.
  • Strong knowledge of community resources and healthcare systems.
  • Excellent communication and interpersonal skills.
  • Valid Driver’s License with an acceptable driving record.
  • Ability to travel within Washoe, Lyon, Carson, Storey, and Douglas Counties.
  • Experience with electronic medical records preferred.
