Care Manager (RN, BSN or MSW)

Our company is growing and we are looking for a dynamic RN, BSN with a Master’s degree, preferably in nursing or MSW, related social service fields will be considered. Nurse Practitioners need not apply. Come grow with us!

• Case Management experience a plus.
• Must live within 20 miles of The Villages.
• Must have community experience and knowledge of community resources.
• Marketing experience is a plus.
• Flexibility is a must.
• Weekend on call once a month is required.
• Travel in Central Florida is required. Must have reliable transportation.
• This is a part time with possibility of full time position including travel
• Ability to work 20 hours+/week
• Pay will be discussed upon interview

Overview of Responsibilities:

• Conducting Assessments and developing/implementing individualizes care plans
• Coordinating care and services
• Monitoring progress by conducting regular site visits and accompanying clients to medical appointments
• Educating and supporting clients and their families
• Documentation and maintaining records

Job Type: Part-time

Pay: $30.00 - $40.00 per hour
