Admissions Intake Coordinator

SUMMARY: The Admissions Intake Coordinator is responsible for managing the admissions process, ensuring a smooth transition for new clients or patients, and coordinating necessary information and services for all Goldwater Care Facilities.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Client Interaction: Serve as the first point of contact for prospective residents, greeting them and guiding them through the intake process. This includes answering inquiries and addressing concerns.

  • Information Gathering: Collect essential demographic, financial, and medical information from clients and co-workers to ensure accurate and compliant registration. This may involve conducting initial screenings or intake interviews.

  • Documenting Management: Prepare and maintain resident charts and records, ensuring all necessary paperwork is completed accurately and in compliance with regulations. This includes entering data into digital systems.

  • Collaboration: Working Closely with other healthcare professionals. Such as case managers and social workers, to coordinate care and services for residents.

  • Clerical Duties: Perform various clerical tasks, including filing, copying and faxing, to support the administrative functions of the intake process.

  • Performs other duties as assigned.

QUALIFICATIONS:

  • Must possess, as a minimum, a high school diploma or GED.

  • Be knowledgeable with Microsoft Office (Outlook, Word, Excel).

  • Must have patience, tact, cheerful disposition and enthusiasm, as well as must be willing to handle residents, staff, and visitors, based on whatever maturity level at which they are currently functioning.

  • Attention to Detail: Meticulous record-keeping and attention to detail are crucial for maintaining accurate resident information.

PHYSICAL DEMANDS:

  • Must be able to move intermittently throughout the workday.

  • Must be able to cope with the mental and emotional stress of the position.

  • Must be able to push, pull, move, and/or life a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet.
