Location: Aurora, OH - Local Long-Term Care Facilities (Community-Based)We are seeking a Licensed Professional Clinical Counselor who is passionate about providing compassionate, evidence-based behavioral health care to older adults and medically complex individuals. This role offers more than a traditional counseling position — it provides clinical autonomy, consistent weekday scheduling, and strong operational support so you can focus on patient care and outcomes.Responsibilities
- Provide individual, family, and group therapy to residents in long-term care settings
- Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning
- Support residents experiencing depression, anxiety, grief, trauma, adjustment to illness, behavioral disturbances, and substance use concerns
- Deliver brief, goal-oriented behavioral interventions aligned with geriatric best practices
- Collaborate with nursing, social services, rehabilitation, and medical providers to coordinate care
- Participate in care planning meetings and interdisciplinary case discussions
- Maintain timely and accurate documentation in the clinical documentation system
- Follow ethical, regulatory, and confidentiality standards
- Build professional relationships with facility staff and serve as a behavioral health resource
- Active, unrestricted LPCC license
- Valid driver’s license and reliable transportation
- Strong written, verbal, and interpersonal communication skills
- Ability to work independently while collaborating with a clinical team
- Comfortable working within long-term care environments
- Commitment to timely documentation and high-quality patient care
- Experience with older adult populations preferred
