About Position: The Home Care Coordinator provides technical, organizational and interpersonal skills necessary to efficiently and effectively coordinate the scheduling, communication and billing documentation of all aspects of the plan for home care. Promotes and maintains health of enrollees in the community through teaching, counseling and appropriate preventive and rehabilitative services. Responsible for assessment of health needs and contributing to the plan of care, initial implementation of nursing care plans and providing nursing care, as well as periodic re-evaluation of individual and family nursing needs. Demonstrates the knowledge and skills necessary to assess, plan care for and provide service to frail elderly enrollees according to assigned responsibilities and (PACE Program) standards. Works under the supervision of the Home Care Supervisor or nursing designee, but applies nursing knowledge and skills per NCBON scope of practice. Utilizes critical thinking in clinical problem solving and decision making relative to cope of practice, knowledge, competency and experience. Participates in call rotation. Performs home visits, end of life/palliative (aka Pathways), and nursing care per nursing protocol. Completes professional performance requirements annually.
- Work Location: 163 Chatham Business Drive, Pittsboro, NC 27312
- Schedule: Monday through Friday, 8:00am – 5:00pm. Please note that his role also requires after hours availability on a rotational basis for urgent home visits between the hours of 8:00am -5:00pm on the weekends.
- Required Travel: 95% - Must be comfortable traveling from worksite to participants home in order to conduct assessments.
Responsibilities & Duties
- Assesses, using the nursing process, the home care needs of a frail/elderly population according to written guidelines, policies and forms
- Identifies and develops specific plans of care.
- Initial and periodic assessments to be conducted minimally, every six months, and must be completed prior to the scheduled team meeting to evaluate safety in the home of the participant.
- Assists in the coordination of 24-hour care delivery and the implementation of all homecare services to ensure that quality services are provided to meet participant needs.
- Establishes and maintains collaborative working relationship with contracted home care providers, ensuring the quality of home care services, that care plans are up-to-date and implemented and followed accurately, and documentation is maintained.
- Assists in the coordination of all nursing care for participants in the nursing home, according to program standards.
- Conducts home safety assessments prior to enrollment according to written guidelines, policies and forms.
- Assesses the participant’s needs, develops the teaching plan, and evaluates the effectiveness of the teaching and counseling provided. Makes referrals to appropriate sources.
- Conducts audits of contracted home care providers and nursing facilities on an annual basis.
- Provides backup nursing care at the day health center as needed per nursing protocols.
- Provides home visits and end of life/palliative (aka Pathways) care per nursing protocols.
- If an RN, participates in call rotation with additional compensation.
Minimum Qualifications
Education/Experience: Graduate of an accredited school of nursing. Required: 2 years of experience as a nurse. Preferred: 3 years nursing experience and has practiced nursing in the last three years. Prefer experience in home care or long term care setting, ideally PACE. RN required. One year of experience with the frail or elderly preferred.

- Meet a standardized set of competencies for the specific position description established by PHSC and approved by CMS before working independently
Type of Employees Supervised: None. Responsible for audits of home care agencies supervision of their employees.
Licensure, Registry or Certification Required: A RN currently licensed in NC; presents credentials which demonstrate ability to meet performance criteria for the position. Must have a valid driver license and be able to drive to and from the home of participants.
Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.
Pay Range: $71,088 - $95,536 (commensurate with years of experience)
