Job Summary
We are seeking a compassionate and proactive Resident Care Coordinator to join our dynamic team dedicated to enhancing the quality of life for our residents. Duties

  • Develop and oversee individualized care plans tailored to each resident’s unique needs, including assistance with activities of daily living (ADLs), medication management, and behavioral support.
  • Coordinate with nursing staff, social workers, families, and external healthcare providers to ensure comprehensive resident care.
  • Assist residents with mobility using equipment such as Hoyer lifts or transfer belts, ensuring safety during movement or transfers.
  • Monitor vital signs, administer medications as prescribed, and document all medical information accurately within electronic medical record (EMR) systems.
  • Support residents with specialized needs such as Alzheimer’s care, dementia management, memory care, autism support, or hospice care.
  • Facilitate engaging activities that promote social interaction, mental stimulation, and overall well-being while managing behavioral challenges through effective behavior management strategies.
  • Maintain a clean and safe environment by assisting with light housekeeping tasks like meal preparation and ensuring adherence to health standards including HIPAA regulations.
  • Drive residents to appointments or outings when necessary, ensuring safe transportation practices.
  • Respond promptly to emergencies by applying first aid skills and coordinating with emergency services as needed.
  • Document incident reports, care plan updates, and communication logs diligently to ensure continuity of care.

Skills

  • Proven experience in assisted living facilities, nursing homes, or home care environments with a focus on senior or disability care.
  • Strong understanding of assisted living procedures, Alzheimer’s/dementia/memory care techniques, and working with individuals with developmental disabilities or autism.
  • Excellent communication skills for collaborating with residents’ families, healthcare teams, and external providers.
  • Ability to assist with ADLs such as bathing, dressing, grooming, feeding, and mobility support using appropriate equipment like Hoyer lifts.
  • Knowledge of medical terminology, medication administration protocols, vital signs monitoring, and HIPAA compliance.
  • Proficiency in EMR systems for documentation and case management purposes.
  • Capable of handling conflict management situations with patience and professionalism while maintaining a positive environment.
  • Physical ability to perform heavy lifting when necessary and maintain safety standards during resident transfers or activities requiring physical support.
  • Valid driver’s license for transportation duties; experience in caregiving roles such as hospice or home health is highly valued.
