Job Title: Care Manager

Location: Remote - Illinois, United States

Type: Full Time


Our Client is seeking a dedicated Care Manager to join their team and support members through coordinated, high‑quality care across the full continuum of services. This role works closely with a multidisciplinary team to ensure members receive the right support at the right time—improving outcomes while enhancing the overall care experience.


Key Responsibilities

  • Complete timely assessments and identify members who would benefit from care coordination.
  • Develop and implement personalized care plans in collaboration with members, caregivers, and healthcare professionals.
  • Conduct telephonic, in‑person, or home visits based on program needs.
  • Monitor and document care plan progress, adjusting interventions as needed.
  • Maintain an active caseload with regular outreach and follow‑up.
  • Support smooth integration of medical, behavioral health, LTSS, and community-based services.
  • Facilitate interdisciplinary care team discussions to ensure aligned and coordinated care.
  • Apply motivational interviewing techniques to support and educate members.
  • Identify barriers to care and coordinate appropriate resources.
  • Collaborate with licensed care managers and leadership when required.
  • Travel within the local area as needed (estimated 25–40%).


Required Qualifications

  • At least 2 years of experience in healthcare, care management, or a medical/behavioral health setting (or equivalent experience).
  • Clinical licensure or certification when required by state regulations.
  • Valid driver’s license, reliable transportation, and appropriate insurance for travel needs.
  • Strong understanding of community resources and support services.
  • Excellent communication, problem‑solving, and critical‑thinking skills.
  • Ability to work independently, adapt to diverse environments, and manage multiple priorities.
  • Proficiency with Microsoft Office and related software.
  • A bachelor’s degree in a healthcare-related field may be required depending on state or program guidelines.
