Job Description:

Position Summary

The Clinical Social Worker provides direct clinical services to individuals, families, and groups, addressing behavioral health needs with a focus on culturally responsive, community-based care. This role will support Rafiki's integrated health model, working closely with medical providers, case managers, and community health workers to ensure holistic wellness for clients.

Key Responsibilities

  • Provide individual, group, and family counseling services, including assessment, treatment planning, and crisis intervention.
  • Conduct psychosocial evaluations and develop culturally competent treatment plans tailored to clients' needs.
  • Support clients in navigating community resources, social services, and healthcare systems.
  • Maintain timely and accurate clinical documentation consistent with professional standards and funding requirements.
  • Collaborate with Rafiki's multidisciplinary team to coordinate holistic care and referrals.
  • Facilitate psychoeducational workshops, support groups, and community healing circles.
  • Provide advocacy and case management for clients dealing with housing, employment, benefits, and other social determinants of health.
  • Participate in regular clinical supervision, staff meetings, and professional development.
  • Uphold Rafiki's mission of advancing health equity, racial justice, and culturally grounded practices.


Requirements:

Required Qualifications

  • Master's degree in Social Work (MSW) from an accredited institution.
  • Current licensure in California as an LCSW (Licensed Clinical Social Worker) or registration as an ASW (Associate Clinical Social Worker) under supervision.
  • Minimum 2 years of experience providing behavioral health services, preferably in community-based or nonprofit settings.
  • Demonstrated commitment to working with Black/African American and other marginalized communities.
  • Strong knowledge of trauma-informed care, racial equity frameworks, and culturally competent practices.
  • Excellent interpersonal, communication, and documentation skills.

Preferred:

  • Experience working in integrated care settings alongside medical and wellness providers.
  • Familiarity with San Francisco community resources and public health systems.
  • Bilingual in Spanish, Cantonese, or another language commonly spoken in San Francisco communities.


Additional Information:

Work Environment & Schedule

  • Full-time, exempt position.
  • Hybrid work schedule with required in-person clinical and community-based services.
  • Some evening or weekend hours may be required for community events and groups.
