CASE MANAGER

Job Summary

Case Manager: Ascendant New York is a free-standing boutique detox, inpatient rehabilitation and outpatient facility located in Midtown Manhattan. Ascendant New York is licensed by the State of New York’s Office of Alcoholism & Substance Abuse Services and is looking for a full-time Case Manager. Must have at least 1 year experience in alcoholism and drug treatment and having a CRPA-P, CRPA, CASAC-T, CASAC, MSW, or MHC strongly preferred. This position will be located at our Inpatient Program. The schedule we are looking to fill is Tuesday through Saturday 9:00 am - 5:30 pm.

Responsibilities and Duties

Responsibilities include:

1) Providing case management services to clients;

2) Hold case management sessions;

3) Chart and document patients’ progress;

4) Case managers will participate in discharge planning, referrals, care coordination with outside service providers, care coordination with internal service providers, involve patient significant others if indicated in treatment, provide crisis intervention, participate in implementation of client recovery plans;

5) Participate in interdisciplinary team meetings;

6) Additional duties will be determined by Assistant Program Manager.

Qualifications and Skills

Must have at least 1 year experience in alcoholism and drug treatment. CRPA, CRPA-P, CASAC, CASAC-T, MSW, LMSW or LMHC strongly preferred.

Job Type: Full-time

Pay: $65,000.00 - $75,000.00 per year

• High school or equivalent (Required)

Experience:

• SUD treatment: 1 year (Required)

License/Certification:
