The Willows at Ramapo Rehabilitation and Nursing Center is seeking a compassionate Social Worker to support residents and families in a skilled nursing environment.

Social Worker Responsibilities include:

• Psychosocial assessments and resident/family support

• Discharge planning and coordination

• Participation in care plan meetings

• Documentation and regulatory compliance

Social Worker Qualifications:

• BSW or MSW required

• SNF or long-term care experience preferred

• Strong communication and organizational skills
