Position Summary:
The Care Coordinator is a vital team member responsible for supporting clients as they navigate pathways to stability and independence. This role involves managing a caseload of approximately 20 clients, working collaboratively with the Enhanced Care Management (ECM) team, the Housing Specialists and maintaining an approachable and supportive presence onsite. Care Coordinators meet weekly with clients to monitor progress and provide ongoing guidance and advocacy.

Key Responsibilities:

  • Client Engagement and Assessment:
  • Build rapport with clients to foster trust and open communication.
  • Conduct intake assessments to identify clients’ needs, strengths, and goals.
  • Collaborate with clients to create individualized action plans addressing housing, sobriety, mental health, physical well-being, and social support.
  • Care Coordination and Advocacy:
  • Connect clients to community resources, including housing, healthcare, mental health services, and job training programs.
  • Work closely with the ECM team to align client care plans with treatment goals.
  • Work closely with Housing Specialist team to help ensure clients are staying on track with long term housing goals.
  • Be available onsite to clients in a supportive and approachable manner, addressing questions and providing timely assistance.
  • Goal Monitoring and Support:
  • Meet weekly with clients to review progress and adjust action plans as needed.
  • Provide motivational support, crisis intervention, and de-escalation strategies when challenges arise.
  • Encourage and guide clients in setting and achieving SMART goals.
  • Documentation and Reporting:
  • Maintain accurate and timely records of client interactions, progress, and outcomes.
  • Prepare reports for program evaluation and compliance with organizational standards.
  • Team Collaboration:
  • Work closely with other team members, including housing specialists, case managers, and program directors.
  • Participate in regular team meetings to share updates, challenges, and best practices.
  • Contribute to the continuous improvement of care coordination services.

Qualifications:

  • 2+ years Experience working with individuals facing homelessness, mental health challenges, or substance use disorders.
  • Strong interpersonal and communication skills, with the ability to build trust and motivate others.
  • Knowledge of trauma-informed care, motivational interviewing, and conflict resolution techniques.
  • Ability to manage multiple tasks and prioritize effectively in a fast-paced environment.
  • Proficiency in maintaining accurate and confidential records.
  • Familiarity with local community resources and support systems.
  • Computer skills and literacy are a MUST

Preferred Qualifications:

  • Background in social work, counseling, or a related field (Bachelor’s degree preferred).
  • Experience with Case Management/ Care Management or similar programs.
  • Peer Supports Certification
