Location: Based in Pleasant Hill, CA - Serving Clients Throughout Alameda and Contra Costa County (East Bay)

Schedule: Full-Time, Monday-Friday (some flexibility required)

We're Comfort Keepers, an award-winning in-home care agency serving seniors across Contra Costa and Alameda County. We're looking for a high-performing External Care Coordinator (ECC) to join our team. If you're a natural problem solver, confident on your own in the field, and passionate about helping others live safely and with dignity, we want to meet you.

What You'll Do:

Establishing and maintaining relationships with care partners
Client intake, prospect management
Schedule and conduct in-home care assessments with new clients
Create and update high-quality, individualized care plans
Reassess care as needs change and communicate updates clearly
Collaborate with the office team to match the right caregiver to each client
Help with onboarding new caregivers and reinforcing safe care practices
Plan and structure your day efficiently
This is a field-based role requiring independence and good time management
Document everything clearly, quickly, and accurately

What We're Looking For:

Minimum 1 year of experience in in-home senior care (non-negotiable)
Experience creating thorough, detailed care plans based on client assessments in a timely manner
Strong verbal communication and documentation skills
Confident working independently in the field
Organized, proactive, and good with time management (time-blocking skills a plus)
Comfortable using Google Suite (Gmail, Calendar, Meet, etc.)
Fast typing and strong digital organization skills
Experience using WellSky (formerly ClearCare)
