Position Summary:
Responsible for all coordination of activities and overall facilitation of clinical flow of patient throughout the course of treatment. In addition to Clinical Counselor job duties, serves as resource and subject expert to Clinical Counselors. Overall coordination of a patient’s care while in residential services, including integration with family services, sessions, communication with referral sources, assessment of patient progress, and communication to the clinical team regarding special treatment needs and procedures.

Essential Job Functions:

  • Completion of patient assessments such as bio psychosocial assessment, patient goals and treatment planning.
  • Utilize multi-disciplinary treatment team to determine appropriate level of care and identify behavioral health needs that may require external placement outside of Brookdale Premier Recovery.
  • Facilitate group therapy and individual counseling.
  • Recognizing, assessing, determining, and evaluating symptoms of addictions according to ASAM and DSM V criteria.
  • Work with family therapist to assess family dynamics, recommend family education programs and ongoing family therapy.
  • Work with Patient Experience Managers to develop a comprehensive continuing care plan that includes input from clinical referents, internal referents, multi-disciplinary treatment teams, family, and patient.
  • Work within the ethical constraints of a certified addiction counselor and other professional or organizational ethical standards as they apply to the position.
  • Document treatment plans, progress notes, treatment plan updates, case consultations, phone calls, and discharge summaries in the patient medical record in accordance with Brookdale Premier Addiction Recovery policies and procedures.
  • Complete necessary utilization review information.
  • Responsible for the development of the treatment plan, goal prioritization, and can concisely document therapeutic and clinical issues to be addressed in treatment and continuing care.
  • Contacts, oversees and coordinators all communication involving patients, referent, and family as appropriate in assessment and care planning processes.
  • Develops and coordinates all individualized care and continuing care plans that include input from the treatment team as well as engagement of the patient, referent, family, and other appropriate collateral resources.
  • In coordination with Family Counseling team, facilitates family counseling session.
  • Conducts individual and group counseling sessions congruent with 12-step program to motivate and support patient awareness, understanding, and motivation to change related to addiction and recovery issues and is responsible for fully integrating the process into the patient’s treatment plan.
  • Communicates and works effectively with a team, consults with the multi-disciplinary team, patient, family, and referent (consistent with CFR 42 and CFR 45) to address clinical issues both verbally and in writing related to the individualized treatment plan, continuing care, including types of care, level of care.
  • Coordinates communication and collaboration with internal and external customers (i.e., referents, business development officers).
  • Must be able to develop and present psychoeducational lectures, specialty groups relative to the disease of addiction, relapse, behavioral addictions and 12 step recovery.
  • Will step in for leadership when unavailable to run meetings, run point for clinical needs, and be willing to make decisions for patient care when direct supervision is not available.
  • Will provide checklist for weekend programming and will aid and assign team members to address ATA risk, patient engagements, etc.
  • Performs other duties as assigned.
Knowledge, Education, & Experience:
  • Bachelor’s Degree required in Human Services or other related discipline. Master’s degree in human services preferred, Licensure preferred.
  • Specialty training preferred (MI, DBT, Art Therapy, MBSR, CPT, CBT).
  • Experience in lifespan developmental issues.
  • Experience in facilitating group therapy.
  • CADC preferred.
