Position Overview:

We are seeking a qualified and caring Nurse to join our team. This role focuses on helping clients stay on top of their health needs by scheduling medical appointments, monitoring medications, and ensuring smooth communication between clients, families, and healthcare providers.

Key Responsibilities:

  • Coordinate and schedule medical appointments for clients.
  • Monitor medication schedules, ensuring clients are taking medications safely and as prescribed.
  • Track and update medical records and appointment follow-ups.
  • Communicate with healthcare providers, clients, and families regarding care needs.
  • Provide basic health education and support to clients as needed.
  • Maintain accurate documentation and ensure confidentiality at all times.

Qualifications:

  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a valid state license.
  • Strong organizational and scheduling skills.
  • Compassionate, patient, and reliable with excellent communication skills.
  • Ability to manage multiple priorities and work independently.
  • Previous experience in care coordination or case management is a plus.
