**Essential Duties/Responsibilities**
Demonstrates critical thinking skills in discriminating between protection of sensitive medical/psychosocial health information in compliance with applicable law and regulation regarding mandated reporting.
Demonstrates the ability to access various community resources, such as: food, housing, and utility assistance, and benefit acquisition for state and government level.
Identifies existing problems, utilizing solving skills in collaboration with participant/designated representative based on plan of care, assessment, and the availability of support systems within the community.
Initiates referrals to appropriate community resources and assist
s participant and designated representative in resource acquisition; identifying steps and possible barriers to acquisition of resources.
Accesses consultation with MSW members of the Interdisciplinary Team (IDT) during team meetings regarding participant care and/or within the supervision process.
Documents care provided and the response of the client/family in a risk-neutral fashion; recognizes the sensitivity of psychosocial data and protects confidentiality of same; submits documentation which reflects third-party payor requirements in a timely fashion as described in agency policy.
Assist with collaboration with applicable PACE departments/programs on the acquisition of materials in preferred language for participant care.
Completion of participant education which includes annual acknowledgement of services received, participant rights, grievance process within regulations established for benefits received (i.e.. Medicare and/or Medicaid).
Participate in community meetings to stay abreast of resources available and process for applying for assistance focused on participant care (i.e. housing authority, utilities assistance, etc.)
Possess, maintain, and convey to staff an accurate and thorough knowledge of all program requirements.
Possess, maintain, and convey to staff above average skillset on tools required to perform job tasks including specialized computer software and medical record documentation.
Possess, maintain, and convey to staff accurate knowledge of, and monitor compliance with, HIPAA and agency confidentiality policies.
Maintain the confidentiality of sensitive information.
Communicate with supervisor daily, and as needed, to report all areas of concern/issues/incidents, and completion of assignments.
Serve as an example of agency Standard of Excellence by exemplifying agency vision and goals.
Demonstrate professionalism in relationships with customers/co-workers/supervisors to build rapport while refraining from inappropriately familiar personal relationships.
Complete and attest to accuracy of payroll records and submit for review and processing according to agency schedule.
