Registered Nurse Care Manager

Duration: 13 Weeks with extension
Shifts: Variable 8 - 4:30 pm or 4 - 10 hour shifts of 7:30 am to 5:00 pm - No Call - Weekend and Holiday Rotation is required

We are seeking an experienced RN Care Manager for a 13-week assignment in York, Maine. This role focuses on coordinating utilization management activities and serving as the link between patients, healthcare teams, and payors during the hospital stay.

Responsibilities

• Conduct patient assessments to identify needs and resources for safe transitions of care.
• Collaborate with and lead interdisciplinary teams in care rounds, conferences, and planning.
• Communicate effectively with patients, families, providers, and payors.
• Coordinate with community organizations for durable medical equipment, insurance authorizations, and transportation.

Qualifications

• Active RN license; Associate or Bachelor degree in Nursing required.
• 3–5 years of clinical experience in medical/surgical nursing, home care, case management, and/or utilization review.
• Must have prior utilization review and discharge planning experience.
• Active BLS required.
• CCM or ACM certification preferred.

Job Type: Contract

Pay: $2,500.00 - $2,600.00 per week
