Section 1: Overview

Implement standards of care for medical social work services
Assess the psychosocial status of patients and families/caregivers
Carry out social evaluations and provide counseling as needed
Maintain clinical records and provide information and referral services

Section 2: Qualifications

Graduate of a master's program in social work accredited by the Council on Social Work Education
Minimum of one year's experience in health care, hospice experience preferred
Good verbal and written communication skills
Possess and maintain current CPR Certification
