Position Profile
Provides professional counseling and in depth education about grief and loss to support those doing the work of mourning.

Responsibilities
§ Responsible for delivery of Bereavement Services to survivors following the death of hospice patients, including Spanish speaking survivors, throughout Maricopa County.
§ Assist in the development and facilitation of bereavement support groups in the community including the Hispanic community, as needed/requested.
§ Develops and maintains therapeutic relationships.
§ Provides comprehensive bereavement assessments.
§ Develops and maintains comprehensive plan of care.
§ Provides effective therapeutic counseling and support.
§ Contributes to HOV and the community with grief expertise and support.
§ Creates timely and accurate documentation.
§ Adheres to HOV standards and facilitates continuously improved processes/services.
§ Maintains and enhances professional skills.
§ Adheres to high standards of personal and professional conduct.

Minimum Qualifications
§ Master's degree in counseling, social work or other allied health field from accredited program.
§ Minimum of 1year recent experience in counseling individuals and/or groups regarding loss.

Preferred Qualifications
§ Bi-lingual preferred
§ Hospice experience preferred.
§ LPC, LMFT, or LCSW licensure preferred.
