Wellness Hospice and Palliative Care is a professional, agile and innovative company looking to add to their growing team. We service patients in senior care settings as primary care providers and palliative care providers as well.

The Medical Social Worker is a part of the care team, and is responsible for providing social work services to patients and families to help them cope with their illness and the surrounding circumstances. The Medical Social Worker will need to have knowledge of hospice and palliative medicine and be proficient in patient care techniques.

Responsibilities:

• Communication with families and caregivers.
• Providing care to patients dealing with illness and other life stressors such as divorce, abuse, financial problems, and unemployment.
• Provide advice and referrals for patients for treatment.
• Connect patients with support networks for their family and community.
• Providing counseling to patients and families.

Qualifications:

• Minimum 2 years of experience as a social worker, preferred.
• Active patient contact within past three years, preferred.

Job Type: Full-time

Pay: $65,000.00 - $72,000.00 per year
