Position: Behavioral Health Care Manager (BHCM)

Location: Erie, PA & Ashtabula, OH or Erie, PA & Clymer, NY
Accountability Summary:
LECOM Behavioral Health is looking for a full-time, dynamic master level Behavioral Health Care Manager (BHCM) to join our multidisciplinary healthcare team. This position will require travel 2-3 days per week between Erie, PA and either Ashtabula, OH or Clymer, NY. The BHCM plays a critical role in providing coordinated, patient-centered care between individuals in underserved and rural communities to their healthcare provider. The BHCM is responsible for conducting comprehensive assessments, delivering brief behavioral interventions based on evidence-based practices, and developing individualized treatment and safety plans. Working closely with the primary care provider (PCP) and psychiatric consultant, the BHCM initiates and adjusts treatment plans, monitors patient progress, and ensures continuity of care. This role emphasizes proactive engagement, symptom monitoring, and collaborative care planning to improve patient outcomes.
This position is partially grant-funded through September 2029. Continued full-time employment is contingent upon the availability of funding and/or positions within the service line.
Primary Responsibilities:
- Care Coordination and Outreach: Serve as a liaison between patients and healthcare providers by conducting outreach to engage vulnerable populations. Assist patients in navigating complex healthcare and social service systems to ensure access to appropriate care and support.
- Clinical Intervention and Consultation: Administer validated screening and assessment tools for common mental health conditions. Deliver brief, evidence-based behavioral interventions under the guidance of the treating physician. Collaborate with the psychiatric consultant and primary care team to review patient progress and modify treatment plans as necessary.
- Resource Navigation: Provide resource coordination and facilitate referrals to community services, social support systems, and other external resources to promote holistic and comprehensive care.
- Data Collection and Reporting: Systematically collect clinical data during each patient interaction and at designated intervals. Enter information into the care registry to generate reports that support measurement-based treatment, track progress toward treatment goals, monitor medication adherence and side effects, and evaluate overall health outcomes.
Education:
- Master’s degree in social work, counseling, psychology, or other generally recognized clinical discipline) that includes a clinical practicum OR is licensed in the above clinical disciplines.
Experience/Qualifications/Requirements:
- Clinical experience in behavioral health, including assessment and brief, evidence-based interventions.
- Experience working with adults with mental health challenges required.
- Strong collaboration skills and ability to work effectively within a multidisciplinary care team.
- Demonstrated a cultural competence and commitment to serving underserved and rural populations.
- Excellent interpersonal, communication, and patient engagement skills.
- Proficient in data collection, care documentation, and use of electronic health records.
- Resourceful and organized, with the ability to manage caseloads and coordinate community services.
- Comfortable using measurement-based care to guide treatment decisions and track outcomes.
- Self-motivated, adaptable, and resilient in dynamic healthcare settings .
- Must own a reliable vehicle and current driver’s license.
- Clean background checks (Criminal Record, Child Abuse and FBI) and driving record is required
- Completion of Mandated Reporting within 30 days of hire.
Preferred:
- A current PA LPC or LCSW license.
- Three (3) years of experience in a behavioral health, medical, or human services setting.
- Experience working in a Collaborative Care Model (CoCM).
- Experience working with older adults.
