POSITION SUMMARY
The Intake Coordinator is responsible for providing and coordinating crisis intervention and initial assessments for youth and families interested in Ozone House services. Additionally, the Intake Coordinator is responsible for ensuring that youth and families experience welcoming, responsive, and helpful introduction & orientation to Ozone House, so that continued engagement may be optimized. This position is also responsible for the provision of case management and counseling services to homeless and runaway youth and their families.
EDUCATIONAL REQUIREMENTS
Master's degree in human services or related field
Licensure in the state of Michigan (preferred)
EXPERIENCE / SKILL REQUIREMENTS
2+ years’ experience providing assessment &/or Counseling services to youth & young adults
At least one year of applicable experience in youth/young adult welfare, mental health or related area, working with children and families
Prior experience working with diverse cultural backgrounds.
Must be willing to work in clients’ homes and communities.
Knowledge of and adherence to cultural competency guidelines
Valid Michigan Driver’s License
KNOWLEDGE REQUIREMENTS
Working knowledge of office management practices, including record-keeping and administrative support
Familiarity with document preparation, editing, and filing systems
Understanding the structure, governance, and operations of non-profit organizations & services
Familiarity with non-profit regulations, compliance, and reporting requirements
Awareness of knowledge of the issues and challenges faced by underrepresented individuals
Proficient in communication strategies for internal and external audiences
Awareness and appreciation of cultural diversity, and the ability to work with individuals from different cultural backgrounds, understanding the impact of culture on individuals and communities

ESSENTIAL JOB FUNCTIONS
ASSESSMENT DUTIES
Complete intakes and assessments within a time frame that meets the urgency of client needs.
Provide crisis intervention and referrals for youth and families not appropriate for ongoing Ozone House services. Follow up with referrals to ensure appropriateness and usefulness of referral.
Complete all relevant intake paperwork including required assessments, releases of information, and consent to services.
Complete data entry of intake within 24 hours of appointment completion.
Assess needs and eligibility, and facilitate referral or placement of clients seeking emergency, transitional, or independent housing assistance.
Provide relevant and necessary information regarding new clients to assigned program staff, including attending residential team meetings to present youth interested in admission to the programs.
CRISIS COORDINATION DUTIES
In conjunction with the crisis line leadership, ensures the environment in which Crisis Intervention Services are delivered is safe, appropriate and welcoming to youth and families on an ongoing basis.
Work with crisis line leadership to develop and maintain a comprehensive resource manual, to ensure that appropriate referrals are provided to youth and families accessing crisis line services.
Provide direct consultation and modeling to crisis intervention counselors, by offering written and verbal feedback to and answering crisis calls in the presence of crisis intervention counselors.
Provide immediate telephone and in-person assessments and referrals—including follow-up—services to youth and families in crisis.
Provide timely and relevant supervision to social work interns that supports growth and renewal, the acquisition of self-knowledge, the integration of best practices in crisis intervention and assessment in a manner that recognizes the emotional and personal demands that working with high-risk youth and families' places on professionals.
Work to ensure all crisis intervention processes and procedures meet agency standards of responsiveness, flexibility, and quality.
Provide aspects of onboarding and training to new crisis intervention counselors, as needed.
Oversee Follow-up call process to ensure timely and effective tracking of Ozone House outcomes
CASE MANAGEMENT DUTIES
Provide short-term case management services to homeless youth & their families, including resource information provision, referrals, advocacy, and other assistance as needed.
Develop and maintain Service Plans with each client and/or family.
Complete ongoing documentation for each case assigned including individual Service Plans, progress notes, review forms, and discharge summaries in accordance with agency policies and procedures.
MEETINGS & ADDITIONAL DUTIES
Maintain the on-call phone, on a rotating basis, to provide 24-hour emergency assistance and crisis intervention to homeless and runaway youth.
Maintain a schedule to be present to complete intakes and assessments at both Ozone House locations.
Participate in team meetings as scheduled.
Participate in weekly clinical supervision.
Attend Ozone House all-staff meetings.
Coordinate and chaperone youth activities and/or client events on a rotating basis.
All other duties as assigned
WORK ENVIRONMENT, PHYSICAL DEMANDS AND TRAVEL
This job operates in a professional office environment. The physical demands of the position include vision, effective speech and hearing for extensive telephone contact; repetitive motion; driving or riding in a motor vehicle; standing, sitting, walking, bending, reaching, and stretching; and lifting up to twenty-five (25) pounds unassisted. This role routinely uses standard office equipment with prolonged periods of sitting at a desk and working on a computer. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions, so long as it creates no undue hardship to the agency. Must have reliable transportation to be able to travel and maintain the rigors of a busy schedule.
