What You’ll Do:

• Provide psychosocial support to patients and families in their homes or residential settings.
• Participate in developing and coordinating individualized care plans.
• Support patients facing health, social, or end-of-life challenges with empathy and skill.
• Act as a liaison between patients, families, healthcare providers, and community resources.
• Document care accurately and in a timely manner using our EMR system.

What We’re Looking For:

• Master’s Degree in Social Work (MSW) from an accredited program
• Active Minnesota Social Work License
• Valid driver’s license, reliable transportation, and current auto insurance
• Strong written and verbal communication skills
• Comfortable using electronic documentation tools (training provided)
• Experience in home health, hospice, or medical social work preferred—but not required
