What You’ll Do:
- Provide psychosocial support to patients and families in their homes or residential settings.
- Participate in developing and coordinating individualized care plans.
- Support patients facing health, social, or end-of-life challenges with empathy and skill.
- Act as a liaison between patients, families, healthcare providers, and community resources.
- Document care accurately and in a timely manner using our EMR system.
What We’re Looking For:
- Master’s Degree in Social Work (MSW) from an accredited program
- Active Minnesota Social Work License
- Valid driver’s license, reliable transportation, and current auto insurance
- Strong written and verbal communication skills
- Comfortable using electronic documentation tools (training provided)
- Experience in home health, hospice, or medical social work preferred—but not required
