The Social Service Designee is responsible for planning, organizing and directing the Social Services Department. The Social Service Designee must be able to identify the health related psycho-social needs of residents and shall provide services for these identified needs. The Social Service Designee is directly responsible to the Administrator.

Vulnerable Adult Mandated Reporter

A mandated vulnerable adult reporter of maltreatment, neglect, abuse of vulnerable residents/tenants. Required to report any alleged maltreatment, neglect, or abuse to your supervisor and appropriate state agencies.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

· Conducting pre-admission and admission interviews with the residents and/or family members or both, where appropriate, and providing the necessary materials and input to assure effective admissions. Is responsible for determining appropriateness of prospective resident for facility.

· Communicating to the staff pertinent social service information regarding residents and families who may have social service needs, of which the staff should be made aware. Provides staff with pertinent social data in advance of all new admissions.

· Obtaining and developing social service admissions, summaries and histories and for all residents. Serving on the Quality Assurance Committee.

· Providing discharge coordination service and serving as the discharge coordinator, to include pre-discharge counseling and referral.

· Assisting with resident adjustment to the facility.

· Establishing and maintaining periodic contact with all residents, families and staff, and documenting contacts.

· Responsible for developing social services plans of care and follow-up progress notes. Services on inter-disciplinary care plan team.

· Responsible for determining mode of payment prior to admission for all admissions and is responsible for providing assistance to private pay residents when medical assistance is needed.

· Acting as resident advocate for resident rights and serves as a liaison between staff, families and others.

· Providing counseling services to residents and families when needed.

· Promote/enhance the Health Care Center and community relations.

· Direct and supervise resident and/or family groups, if deemed advantageous to it.

· Serve as resident complaint coordinator.

· Develop and update Social Services Policies and Procedures yearly.

· Provide referral to appropriate resources.

· Coordinates ALL resident room changes when appropriate or necessary.

· Provides at least annual Inservice to educate staff to psycho-social needs.

· Is the joint Admissions Director with full decision making authority.

· Active Treatment and total Plan of Care will be updated as needed but within state and federal guidelines.

· Have desire and ability to work with geriatric, convalescent, chronically, seriously or terminally ill residents and their families with respect, caring, and understanding.

· Adheres to Federal/State regulations and guidelines, including Resident Rights and Confidentiality.

· Must have basic computer skills or be able to acquire such skills.

· Is able to present concise and meaningful reports both verbally and in writing.

· Follows instructions willingly, able to meet deadlines, organizational loyalty, honesty, punctual, and dependable.

· Abides by and assures compliance with facility policies and procedures.

MARGINAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

· The Social Service Director is expected to provide other services not listed under duties when appropriate and the situation deems it necessary. Is expected to support Administration.

· Is expected to attend staffing and staff meetings with the Administrator and serve on other committees if deemed appropriate.

Job Type: Full-time

Pay: $19.56 - $26.00 per hour
