Location: Remote/Member facing: Will be Completing SAI assessments at member home; source from San Antonio, TX.SHIFT: Mon - Fri 8:00 am to 5:00 pm CSTDuration: 6+ monthsJob Description: Performs care management duties to assess and coordinate all aspects of medical and supporting services across the continuum of care for complex/high acuity populations with primary medical/physical health needs to promote quality, cost effective care. Develops a personalized care plan / service plan for long-term care members, addresses issues, and educates members and their families/caregivers on services and benefit options available to receive appropriate high-quality care.Day to day responsibilities:

Completing SAI assessments at member home.
Telephonic outreaches
Care Coordination
Following making/follow up on requests for DME, Therapies, provider services etc.
28 day follow ups
Documentation and forms

Describe the performance expectations/metrics for this individual and their team:

Follow work processes
Complete assessments, outreaches and follow ups with in turn around times.

Required Skills/Experience: Preferred Skills/ Experience:
1. RN with experience in pediatrics 1.
2. Knowledgeable of word ,excel, outlook and teams 2.
3. Communication skills 3.
Education Requirement: Associate's Degree Education Preferred: Bachelor's Degree
