Eden Health is hiring a Licensed Clinical Social Worker (LCSW) to provide in-home counseling services to our patients throughout the Greater Idaho Falls and Pocatello areas.

Job Summary

  • Performs ongoing accurate and comprehensive assessments of each patient as ordered to assess the social, spiritual, and emotional needs of the patient and family related to the problems of serious illness;
  • Obtains necessary provider orders and collaborates with patient’s primary care provider and palliative care team as the patient's needs dictate;
  • Develops and implements care plans in collaboration with the patient’s primary care provider and palliative care team;
  • Offers counseling and crisis intervention services;
  • Facilitates family meetings and provides liaison function between palliative care team and community hospice, home care agencies, long-term care facilities and other community services as needed;
  • Assists with advance care planning and decision-making processes;
  • Educates patients and families about illness progression and care options;
  • Connects patients and families with community resources and support services;
  • Provides bereavement support and grief counseling;
  • Performs transfer and discharge planning functions in accordance with Eden policy, applicable laws, regulations, and standards;
  • Completes and submits necessary documentation in accordance with Eden Health policy and applicable regulations;
  • Participates in case conferences;
  • Provides supervision and mentorship to provisionally licensed social workers, bachelor level social workers, assistants and completes supervisory visits as required;
  • Understands palliative care philosophy, hospice philosophy and issues of serious and terminal illness;
  • Participates in Quality Assurance Performance Improvement (QAPI) activities.

Education/Experience Requirements

  • Masters degree from a school of social work accredited by the Council on Social Work Education
  • Licensed Clinical Social Worker (LCSW) certification
  • Clinical Social Worker licensure in the state of practice; if required by state law/regulation
  • Valid driver’s license and reliable transportation
  • Proof of automobile insurance (MVR checked)
  • Proof of current CPR certification
  • Able to pass criminal background check and national sex offender clearance
