Summary:
The position description is only to be used as a guide. It does not constitute any type of contract between the Agency and employee. At the Agency’s discretion, the description may be changed and additional duties, qualifications or requirements may be added at any time.
The Intake Coordinator is contacted by referring physician or Agency and ensures all referrals are processed in a detailed manner. Intake Coordinator ensures nursing needs are communicated to the RN Supervisor for assignment of staff. Communicates effectively with all staff to promote quality care. This individual will demonstrate leadership in the office directing nursing staff on proper response to lab results. Will begin generation of the patient’s care plan at the time of referral. Will act as temporary supervisor in the absence of the nursing supervisor.
This position may be required to crosstrain within the agency. Responsibilities in secondary position may require access to medical records and other computer access exceeding routine computer usage. Access to patient information may be necessary to fulfill the requirements of this role; therefore, adherence to Ballad privacy and security policies and practices is a must.
LPN can verbalize the situations that they should report to the RN each shift.

Requirements:
Must be a graduate of an approved School of Nursing and carry current nursing licensure in the appropriate state. Must have at least one year of nursing experience. Experience in computer, insurance case management, office management, accounting (preferably in home health care preferred. Home health experience required. Home health supervisory experience preferred. Experience in home health marketing or physician liaison experience – obtaining and processing home health referrals and orders preferred. Knowledge of Medicare, private insurance, Medicaid, and other regulatory and documentation requirements for coverage of services necessary. Must successfully complete Agency orientation. Must have own dependable transportation. Must carry current personal collision and liability insurance on any vehicle driven.

Licenses and Certifications:
Must maintain current driver’s license in the appropriate state. Must possess and maintain current certification in cardiopulmonary resuscitation.
