**MUST HAVE HOME CARE EXPERIENCE**

Homebridge Health Care Co. is seeking a dedicated Home Care Coordinator to join our team. This role ensures seamless coordination of home care services by onboarding and offboarding clients, maintaining compliance in client documentation, and managing client schedules. The ideal candidate will be highly organized, detail-oriented, and capable of addressing client and employee concerns related to services.

Key Responsibilities:

• Onboard and offboard clients, ensuring all documentation is completed
• Maintain and update client records to ensure compliance
• Create and manage client schedules in accordance with care needs
• Handle client and employee concerns related to home care services
• Manage client profiles in myEVV and track service authorizations
• Collaborate with caregivers and other staff to optimize care delivery

Job Types: Full-time, Part-time

Pay: $18.00 - $23.00 per hour
