Job Description: Responsible for the appropriate and timely discharge and transfer arrangements for patients referred to the Care Management Dept.
QualificationsEducation: Master’s degree in social work required.Experience: At least one year of actual work experience in a hospital setting or an ongoing plan for consultation between the social worker and a qualified social worker must be developed.
