QualificationsEducation:Master’s degree in social work required.Experience:At least one year of actual work experience in a hospital setting or an ongoing plan for consultation between the social worker and a qualified social worker must be developed. Must have knowledge of social and physical factors that affect functional status at discharge and knowledge of community resources to meet post
