Clinical Care Coordinator – Home Health - LVN – (In Office - Arlington)

Our Clinical Care Coordinator is a clinical (LVN) office position that works Monday - Friday in our Arlington office. Classic Home Care is a fast-paced environment coordinating patient care with our Director of Nursing ensuring communication with all health care partners. This includes assessing patient needs, developing and implementing care plans and coordinating with all stakeholders to maximize efficiency, compliance and effective care.

This is a full-time office-based position in Arlington, Texas.

Duties will include the following:

• Assess patient needs and develop care plans in collaboration with patients, families, and other healthcare providers.
• Work closely with Classic’s clinical field staff to provide the best care for our patients.
• Track and coordinate lab orders and results.
• Coordinate with medical scheduler to review past due visits.
• Coordinate the delivery of home health services, including nursing, therapy, and social work services.
• Monitor patient progress and make adjustments to care plans as needed.
• Provide education and support to patients and their families.
• Willingness to assist with time sensitive home visits.
• Participate in quality improvement activities.
• Educate patients and families on home health care.
• Ensure patients have appropriate medical supplies for treatment.
• Integrates orders from all relevant physicians involve into the plan of care and ensures orders are approved by the responsible physician.

Education and Experience

· · Active LVN License (Required)

· · Nursing experience in a Home Health or Assisted Living setting (Preferred)

· · CPR (Required)

· · Current Texas Driver’s License (Required)
